The Competence Centre Dental Health Mid (TKmidt) accepts referred patients from dentists and dental hygienists in both the private and public dental services. TKmidt also accepts referrals from psychologists and doctors. We have the following specialties/dentists:
- Endodontics
- Jaw and facial radiology
- Orthodontics
- Oral Surgery
- Pedodontics
- Periodontics
- Prosthetics
All patients to be examined and/or treated by a specialist at TKmidt must be referred in writing for the time being.
Referrals cannot be emailed. TKmidt can offer multidisciplinary assessment for the good of the patient.
The citation must contain
- To whom it is referred
- Reason for referring/what do you want TKmidt to contribute?
- Who to refer with contact info.
- Patients belonging to priority groups MUST be referred from public dental clinic
Complete patient information
- Address and phone number
- Social security number 11 digits
- Relevant anamnestic information
- Name of family doctor with contact details
- Group statement/rights in the public dental service or adult paying patient
- Impact point Helfo
- Who should pay if the patient is not going to pay himself
- Patient's medical record number in public clinic
Further information
- Subjective symptoms/clinical findings/diagnosis
- Assessment of the degree of urgency
- Rtg. images must be attached, either on CD or memory stick
When the patient is successfully treated, epicrisis is sent to the referrer
TKmidt does not offer regular dental treatment.
